Pre-Enroll for LINC school programs.

A parent interview is required to complete your students enrollment.

This online form begins the application process.

Completing the online application is the first step, but it does not guarantee admission to the program.

An in-person interview is REQUIRED.

Your LINC Site Coordinator will contact you to arrange the interview.

Sites will hold in-person enrollment events in August prior to the beginning of school. Enrollment takes place at the school unless otherwise noted. Enrollment times are also available by appointment with your site coordinator.

Enrollment is required for participation in the program.

You will need:

  • Child Enrolling: Basic information about the child

  • Parent/Guardian: Basic information including employer, work hours

  • Authorized Pickup/Emergency Contact: Contact information for at least two individuals

  • Medical Information: Doctor/clinic including phone and hospital

What you will need to finish enrolling:

  • Immunization records

  • If your child has a diagnosed medical condition, an ICP (Individualized Care Plan) from your child’s doctor.

  • 5 to 10 minutes for parent interview
